If you are using the Service on behalf of an organisation, you represent that you have the authority to bind that entity to these Terms.
Signed.Health is provided for lawful professional use in connection with digital consent, clinical documentation, and related workflows.
You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account.
You agree to provide accurate and complete information when using the Service.
Signed.Health is a software platform designed to support the documentation of informed consent. It does not provide medical advice, legal advice, or clinical decision-making support.
All clinical responsibility remains solely with the clinician using the Service. This includes, but is not limited to:
• Determining the appropriateness of any procedure
• Conducting a proper consent discussion
• Ensuring the patient understands the risks, benefits, and alternatives
• Tailoring consent information to the individual patient
While Signed.Health may provide template consent forms and suggested risk content, these are provided for general informational purposes only.
The clinician is solely responsible for reviewing, editing, and supplementing any consent form to ensure that all material risks, complications, and alternatives relevant to the specific patient and procedure are included.
Signed.Health does not guarantee that any template, library content, or generated document is complete, accurate, or sufficient for any particular clinical scenario.
The absence of any specific risk, complication, or alternative on a consent form generated or used through the Service shall not create any liability on the part of Signed.Health.
Consent forms available through the Service have been used in a large number of clinical consent procedures; however, past usage does not constitute a representation, warranty, or guarantee of legal adequacy, completeness, or suitability in any individual case.
You acknowledge and agree that the duty to obtain valid informed consent rests entirely with the treating clinician, and not with Signed.Health.
You agree not to:
• Use the Service for unlawful or fraudulent purposes
• Attempt to gain unauthorised access to the system
• Upload malicious code or harmful content
• Copy, resell, or exploit the Service without permission
Some features of the Service may require payment. Fees are billed as agreed at the time of subscription and may renew automatically unless cancelled.
We reserve the right to change pricing with reasonable notice.
You retain ownership of the data you upload. We process personal data in accordance with our Privacy Policy.
While we implement appropriate security measures, no system can be guaranteed to be completely secure.
All rights, title, and interest in the Service (excluding your data) remain the property of Signed.Health or its licensors.
You are granted a limited licence to use the Service in accordance with these Terms.
The Service is provided on an “as is” and “as available” basis.
To the fullest extent permitted by law, Signed.Health disclaims all warranties, express or implied, including but not limited to:
• Fitness for a particular purpose
• Accuracy or completeness of content
• Legal adequacy of consent documentation
• Non-infringement
• Availability or reliability of the Service
To the fullest extent permitted by law, Signed.Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including:
• Failure to obtain valid informed consent
• Omission of material risks or complications
• Clinical decisions made by users
• Patient outcomes or complications
• Loss of data, revenue, or business opportunity
Signed.Health shall have no liability whatsoever arising from or related to any failure to include specific risks, complications, or alternatives in a consent form, whether or not such form was generated, suggested, or stored using the Service.
You acknowledge that:
• The Service is a documentation tool only
• Clinicians remain solely responsible for consent content
• Use of templates without review is at your own risk
To the maximum extent permitted by law, total liability shall not exceed the amount paid in the previous 12 months, or £100 if no fees were paid.
We may suspend or terminate access if you breach these Terms. You may stop using the Service at any time.
We may update these Terms from time to time. Continued use of the Service constitutes acceptance of the updated Terms.
