Signed.Health is a clinician-built platform designed to support high-quality, consistent, and efficient documentation of informed consent.
Signed.Health was created by Dr Magnus Lynch, a UK-based consultant dermatologist and skin surgeon with extensive experience in clinical practice, research, and patient care.
Over many years of clinical work, Dr Lynch developed and refined a comprehensive library of consent forms across a wide range of procedures and treatments. These forms were shaped through real-world use in thousands of patient interactions, evolving to reflect best practice in communication, clarity, and medico-legal robustness.
The forms available within Signed.Health are based on this accumulated clinical experience. They are designed to:
• Support structured, consistent consent discussions
• Improve clarity for patients
• Enhance documentation quality
• Save clinicians time while maintaining high standards
While these forms provide a strong foundation, they are intended to support — not replace — the clinician’s judgement and responsibility.
Signed.Health is a documentation tool. It does not provide medical or legal advice.
Clinicians using the platform remain fully responsible for:
• Conducting an appropriate consent process
• Tailoring information to the individual patient
• Ensuring all relevant risks, benefits, and alternatives are discussed
All templates should be reviewed and adapted as necessary for each specific clinical scenario.
Our goal is simple: to improve the quality, consistency, and efficiency of clinical documentation while supporting better patient understanding and safer practice.
Signed.Health reflects years of hands-on clinical experience, translated into a modern, scalable digital platform for healthcare professionals.
