A practical guide to using Signed.Health for creating, reviewing, signing, and securely storing consent documentation.
Signed.Health is designed to support clinicians in delivering clear, structured, and efficient informed consent documentation. The platform helps generate procedure-specific consent forms that can be reviewed, signed electronically, and securely stored.
Signed.Health supports the consent process but does not replace the clinician’s professional responsibility to explain the procedure, answer questions, and ensure the patient understands the risks, benefits, alternatives, and the option of no treatment.
Signed.Health works with all medical records systems, but has been tested most extensively with Carebit, which is used as the example workflow below.
For the best experience in clinic, Signed.Health is recommended for use on an iPad or iPad mini. These devices provide an ideal format for reviewing consent forms with patients and obtaining electronic signatures.
Go to www.signed.health.com and log in using your account credentials. If you have not used before then please register
Choose the appropriate consent form for the planned treatment or procedure.
Ensure that the selected form matches the procedure being discussed.
Where possible, send the consent form to the patient before the procedure so they have time to review the information.
This allows patients to consider the risks, benefits, and alternatives in advance, and helps improve the quality of the consent discussion on the day of treatment.
• Patient name
• Date of birth or patient ID
• Procedure or treatment details
Review the form before discussing it with the patient to ensure it is appropriate for the planned treatment.
Add any patient-specific information where necessary.
Use the consent form as a structured framework for discussion.
The discussion should include:
• The nature and purpose of the treatment or procedure
• Expected benefits
• Common risks
• Serious or uncommon risks
• Reasonable alternatives
• The option of no treatment
• Any patient-specific considerations
Patients should have the opportunity to ask questions.
Before signing, confirm that the patient understands the information, has had their questions answered, and is making a voluntary decision.
The patient can sign the consent form electronically on the iPad.
The clinician should also sign.
Once signed, generate the completed PDF document and/or save as an image.
Safari on iPad does not always allow the same "Save As" behaviour as desktop browsers. Instead, save the PDF locally to the iPad using the Files app.
It is recommended to create a dedicated folder called consent-forms under On My iPad.
• Tap the Share button
• Select Save to Files
• Under Locations, choose On My iPad
• Create or select a folder called consent-forms
• Tap Save
If On My iPad is not visible, open the Files app, tap Browse, and enable local storage if required.
Do not save consent forms to iCloud or other general cloud storage services, as these may not be approved or certified for storing medical records.
The preferred method of providing a copy to the patient is to securely upload the signed consent form to the patient’s online medical record or patient portal.
Example workflow using Carebit:
• Open the patient record
• Select Letters
• Select Upload
• Choose the appropriate destination folder
• Upload the signed PDF
This allows the patient to securely access their consent document through the approved clinical system.
Once the signed PDF has been uploaded to the patient record, delete the local copy from the iPad.
This helps maintain information security and reduces the risk of retaining patient data unnecessarily on the device.
